Ed Rascati serves as the founder and director of EJR Consulting, Inc. a medical and vocational case management-consulting firm, based in Chicago, Illinois. Ed is a Certified Vocational Rehabilitation Counselor, with over 30 years of experience. Ed's professional experience includes working as a professional rehabilitation consultant with CorVel Corp., Rehabilitation Management, Inc., Ability Management Enterprises, Ltd. and now EJR Consulting, Inc.
Ed possesses a Master of Science degree in Rehabilitation Counseling from Illinois Institute of Technology. In addition to his vocational training and experience, Ed has an extensive medical background, having earned a Bachelor of Science degree in Kinesiology from the University of Illinois. Also, Ed has gained valuable practical medical experience while employed at New Medico Head Injury Systems and with the Rehabilitation Institute of Chicago, working in spinal cord research.
Ed also provides a unique perspective of the world of work, gained from his years working as a manager in a high volume supermarket. He understands the complex issues that employers and employees often present regarding return to work issues.

Samantha Hoevel-Kujawa, MA, CRC, LCPC
Professional Rehabilitation Counselor
Samantha graduated from Northern Illinois University with a Master’s Degree in Vocational Rehabilitation Counseling. Samantha is a Certified Rehabilitation Counselor and a Licensed Clinical Professional Counselor.
Samantha has over 12 years of experience working as a Vocational Rehabilitation Counselor in Worker’s Compensation and Department of Defense /Longshore Base Claims. Samantha conducts Vocational Evaluations, Labor Market Surveys (worldwide) and provides job seeking skills training, resume and interview development and guidance, as well as counsels clients on how to move forward after sustaining an injury. Samantha also has provided expert testimony for depositions and trials.
Samantha has previously focused on the Deaf and Hard of Hearing community along with individuals on the Autistic Spectrum. Thanks to working with a broad range of people with disabilities, Samantha is able to provide a unique approach to rehabilitation counseling that tailors to each person’s strengths.

Jacky Ormsby, MS, CRC, LCPC
Professional Rehabilitation Counselor
Jacky possesses a Master of Science degree in Rehabilitation Counseling from Southern Illinois University. She has over 25 years of experience in the workers compensation field.
Working as a Vocational Rehabilitation Counselor, with Worker's Compensation and Department of Defense /Longshore Base Claims, Jacky conducts Vocational Evaluations, Labor Market Surveys (worldwide) and provides job seeking skills training, resume and interview development and guidance, as well as counsels clients on how to move forward after sustaining an injury. Jacky also has provided extensive expert testimony for depositions and trials. She had her own successful business for 17 years.
